Loose mortar flashing? Improper pattern lay looks very unfinished from the ground. These and other holes in sheathing not repaired by installers. Bad vent hole patch is dangerous and will lead to leaks.
This 2 day job on large roof done in 6 hours. Shortcuts taken during cold weather are common.
